Jasmine Fields, Executive Director at UnitedHealthcare, says it’s important to review your benefits now to make sure you aren’t overlooking options that could make life easier and more affordable.

If you or someone you care for is on Medicare or Medicaid, you may qualify for a Dual Special Needs Plan (D‑SNP). These plans are designed for people who need extra help because of income, disability, or health needs — and nearly half of those who qualify aren’t enrolled yet.

  • Simplified care coordination: Plans often help track doctor appointments, manage medications, and connect members with community services such as housing assistance or caregiver support.
